There will be no charge(s) incurred, unless you NO SHOW or provide less than 24 hr notice of cancellation.

‣ Once your intake is confirmed, you will be sent additional forms that are required prior to your appointment date (consent for treatment, privacy policy, etc.). Your initial visit is subject to cancellation if these are not completed 24 hours before your scheduled appointment.
